Empowering Families

Doors 2 Success has two distinct program areas to develop the knowledge and skills necessary to maintain stable housing. The organization also invests in two financial assistance mechanisms—emergency services and security deposit assistance—to reinforce clients’ abilities to stay in their homes.

For families striving toward economic independence, the critical and stabilizing financial aid provided by Family Services Programs can be the make-or-break difference for success now, and for improvements across generations. Through helping low income, homeless and at-risk families to access and maintain housing, Doors 2 Success keeps children at home, out of foster care, in school and with a firm foundation for flourishing.

Doors 2 Success Community Collaborations

In collaboration with community programs, Doors 2 Success also provides access to education, training, and referrals to other service providers, all tailored to each client’s specific needs. Support for families can also look like tangible goods. For example, this past year, 175 backpacks filled with essential school supplies were given to children of TANF-enrolled families. In 2022, the following outcomes were achieved for 156 families (247 adults and 550 children): 93% of these families retained stable housing, and:

Serving Older Adults

Older adults are often at great risk of poverty, isolation or exclusion from the community, health challenges, and lack of access to health care and other critical services. For those without a social support system, the involvement of a Service- Care Coordinator—who helps navigate resources through education and hands-on support—significantly increases the likelihood of maintaining stable housing and independent living.

Empowering Wellness

Close connection with Service-Care Coordinators allows for the resolution of issues before they arise or worsen. For example, Sandy (photo) now has an updated prescription and a new pair of glasses to better see and enjoy life. To further support its 800 older adult households, Doors 2 Success partners with Rocky Mountain Health Plans (United Health Care) to streamline and augment health care coordination services.

In 2022, 57% of eligible households took advantage of service-care coordination and experienced:

Increased involvement in personal health and wellness, e.g., attending classes for diabetes prevention, medication management

Increased primary care and post-operative follow-up and system utilization, including transportation support and 21 PACE referrals

Reduced emergency room visits, readmission rates, preventable diseases or injuries, and out-of-home residential placements

Reduced Medicaid spending (by 16% in 2022), indicating client needs are being met proactively, avoiding cost for complex or emergency care.
